Chapter 6. Temporary Consulting Permits |
Section 6.03. Application procedure.

- (1) An applicant for a temporary consulting permit shall file a completed application with the board. All supporting documents shall be provided in English. An application is not complete until the board receives all of the following:(a) An application form provided by the board and completed by the applicant which includes the applicant's notarized signature.(c) The fee required under s. 440.05 (6) , Stats.(d) Verification of licensure records and status which has been sent directly to the board by every state or country in which the applicant has ever held a license or certificate to practice veterinary medicine.(e) Written verification from a veterinarian that a consultation is being sought.
