Section 4.02. Application procedure.

- An applicant for licensure by endorsement shall file a completed application with the board. All supporting documents shall be submitted in English. An application is not complete until the board receives:(1) An application form provided by the board and completed by the applicant which includes the applicant's notarized signature.(4) Verification of licensure records and status which has been sent directly to the board by every state or country in which the applicant has ever held a license or certificate to practice veterinary medicine.(5) A certificate of graduation from veterinary college which shall be signed and sealed by the dean of the school and submitted directly to the board by the school.
