Section 152.01. Certificate of registration renewal.

A registrant who desires to renew a certificate of registration shall submit to the department an application form for renewal that is signed by the registrant under penalty of perjury and contains current information on all matters required in an original application form for registration under s. SPS 151.02 and pay the fee specified in s. SPS 152.05 .
