Section 143.08. Definitions.

  • (1)  Participating physicians are licensed physicians who have demonstrated their competence and are certified by the American Board of Otolaryngology, American Academy of Ophthalmology and Otolaryngology, and some others as determined by the CSHCN program.
    (2)  Participating clinical audiologists are those clinical audiologists who have met the requirements of the CSHCN program.
    (3)  Participating clinical audiologists dispensing hearing aids are those clinical audiologists who have met the requirements of the CSHCN program. They shall be licensed in the state of Wisconsin to dispense, fit and service hearing aids.
    (4)  Children with special health care needs program clinical audiologists are those clinical audiologists employed by the department of health services, children with special health care needs program, to administer the Wisconsin state hearing conservation program.
    (5)  Participating hearing aid specialists are hearing aid specialists licensed in the state of Wisconsin who have agreed to supply hearing aids and services pursuant to the above rules.
    (6)  The program for children with special health care needs' list of participating physicians, clinical audiologists, and hearing aid specialists are those physicians, clinical audiologists and hearing aid specialists who have agreed to work with the CSHCN program pursuant to these rules.
    (7)  Parent includes legal guardian of child.
