Section 143.01. Eligibility requirements for a hearing impaired child in need of amplification and services.

(1) The child shall be under 21 years of age. Children over 16 years of age may be referred to vocational rehabilitation for services.

(2) Diagnosis shall be established by a physician who is on the children with special health care needs (CSHCN) program list of participating physicians.

(3) Further hearing assessment and hearing aid evaluation shall be completed by a clinical audiologist who is on the CSHCN program list of participating clinical audiologists.

(4) Financial eligibility shall be determined by the latest standards as developed by the CSHCN program.
